MISSION:
 
Yavapai County Community Health Services will provide leadership, information and services that contribute to improving the health and well-being of Yavapai County residents.
 
AGENCY SELF-DESCRIPTION:
 
Provide comprehensive support and services to pregnant women, their infants and children and all family members. A variety of primary care, home visiting, case management services are available.

MEDICAL CARE/PRIMARY CARE
Medical care is provided through our federally funded primary care clinic, including immediate and chronic care, immunizations, dental services, and mental health services. Sliding fee scale is available and most insurance accepted. Three convenient sites located county-wide.

CHILDHOOD IMMUNIZATIONS
Immunizations for vaccine-preventable disease (Measles, Mumps, Polio, Diphtheria, Pertussis, Hepatitis and more) are provided to children at the appropriate age intervals. A $20 per child administration fee is charged. No one is denied services based on inability to pay the administration fee.  Most insurances accepted. Immunizations are available at all three sites listed above.

STD TESTING
Low or no cost STD testing is available for teens and adults, male/female.  No parent required if at least 13 years old.

WIC (Women, Infants & Children Supplemental Nutrition Program)  
The WIC program provides nutritious foods, education about healthy eating, and community referrals. WIC serves pregnant, breastfeeding, and postpartum women, and infants and children under age five who meet WIC eligibility. Women participating in WIC also receive breastfeeding information and support through the Breastfeeding Peer Counselor Program. HOME VISITING PROGRAMS
 
HEALTH START
Health Start is a wonderful home visiting program that connects pregnant women and/or women with a child under the age of 2 to an experienced Community Health Worker (CHW).  Home visits are done to provide education and support about health, development, and safety.  During home visits, the CHW will answer your questions about yourself and your child.  The CHW can help to connect you with other needed services. In the Prescott/Quad Cities area call (928) 442-5478; In the Verde Valley call (928) 634-6849.
