For Web referral: http://www.azdes.gov/azeip

MISSION:

We are dedicated to the belief that all children are capable of incredible growth despite any limitations they may presently be demonstrating. To this end, we strive to work with Yavapai County families with children (birth to 3) to provide the best possible early intervention services available, without regard to degree of delay, income, or race.

AGENCY SELF-DESCRIPTION:

High Country Early Intervention, Inc. is contracted with AzEIP to provide Early Intervention services in a team-based approach to families, throughout Yavapai County, with children birth to age 3 years who experience developmental delays or disabilities in accordance with State eligibility criteria. The program accepts referrals from a variety of referral sources (families, physicians, Community Health Nurses, CPS workers, etc.). Anyone can make a referral. To make a referral for your own child or another child contact High Country Early Intervention, Inc. via one of the above methods. All referrals, no matter where the child lives in Yavapai County, go through HCEIP in Prescott. Referral, screening, evaluation and, if the child is found eligible, Individual Family Service Plan (IFSP) development is at no cost to the family. The IFSP is developed to identify needed services and supports to address a family’s priorities, needs, and concerns related to supporting their child’s development during daily activities. Ongoing services are provided by Developmental Specialists, Physical Therapists, Occupational Therapists and Speech Therapists. Families may be responsible to pay for a portion of the child’s services, depending on income and family size. Many families find their percent to pay is zero.
